Job Description:
This is a 30 hour/week role with the option of being 40 hours/week if the ideal candidate prefers a full time schedule.
This position encompasses the following duties:
-Advocate for patient needs by conducting ongoing psychosocial needs assessments and communicating these needs to the treatment team o Plan and implement appropriate inpatient disposition in collaboration with the treatment team o Effectively identify patient, family, institutional and community barriers to discharge or transitions in treatment.
Coordinate referrals and facilitate transitions in treatment while addressing the barriers identified o Coordinate the development and formulation of psychosocial and case management needs assessments through available resources in the community o Facilitate and/or provide direct clinical therapeutic interventions to prevent relapse and ensure continuity of care based on physician care plans o Provide information/education to patients and their families/significant others regarding illness, recovery and treatment options o Work collaboratively and professionally with the treatment team in maintaining a collaborative relationship with the outside providers and the reimbursement team to optimize the provision of high-quality patient careQualificationsCandidate must be able to speak Spanish o Master's Degree in social work required with a current valid LICSW o Exhibits ability to be competent in the following areas:
o Individual case management and knowledge of managed careo De-escalation/restraint management techniqueso Motivational Interviewing and short-term treatment techniqueso Strong ability to assess and evaluate patient needs and develop treatment plans to address the identified needs.
o Strong organizational skillso Ability to be flexible in a fast-paced work environmento Brief family interventions as requiredFLSA Status:
